Job Responsibility

  • Provides high-quality, compassionate, and ethical nursing care to patients in their homes, across various locations nationally
  • This position will work independently and collaboratively with the interdisciplinary team to deliver care to patients who are rehabilitating, managing chronic conditions, or are in the end stage of life
  • This position requires a highly skilled and adaptable RN with strong clinical, communication, and organizational skills

  • Registered Nurse License

  • Clinical Care: Provides direct nursing care to patients in their homes, following established care plans and physician orders
  • This includes, but is not limited to: assessment, medication administration, wound care, teaching, and emotional support

  • Compliance: Adheres to all applicable federal, state, and local regulations and guidelines, including HIPAA, OSHA, and infection control protocols
  • Maintains confidentiality of patient and employee information
  • Collaboration: Collaborates effectively with physicians, other healthcare professionals, patients, families, and caregivers
  • Participates in care coordination meetings and communicates effectively regarding changes in patient condition
  • Case Management: Manages a caseload of patients, meeting productivity expectations, prioritizing care needs and ensuring seamless transitions of care
  • Proactively identifies and addresses potential issues or concerns
  • Documentation: Completes all required documentation accurately and timely, including but not limited to: electronic health records (EHR), visit notes, care plans, and other required forms
  • Professional Development: Maintains current knowledge and skills through continuing education and professional development activities
  • Actively seeks opportunities to enhance clinical expertise
  • Travel: Willing and able to travel extensively to various locations nationally, as assigned, with flexibility to adapt to different work environments and patient populations
  • Responsible for all travel arrangements and expenses as outlined in the company's travel policy
  • Safety: Maintains a safe work environment for self and patients, adhering to all safety protocols and reporting any safety concerns immediately
  • Professionalism: Maintains a professional demeanor and appearance at all times, representing CommonSpirit Health at Home positively to patients, families, and colleagues
