Job Responsibility

Assume responsibility for triage of after-hours patient and family phone calls, determining the need for services including assessments, symptom management and care plan updates, delegating nursing visits to appropriate staff members
Obtain data on physical, psychological, social, and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team as appropriate
Initiate communication with the attending physician, other interdisciplinary staff members and other agencies for the purpose of coordinating optimal care.
Perform clinical pronouncement of death and provide support to family
Maintain an accurate clinical record through the timely preparation of care plans and clinical and progress notes

Job Responsibility

Ensures the timely and adequate delivery of hospice services to the terminally ill patient and their family
Assesses the total needs of the patient/family during regularly scheduled and after hour home visits
Coordinates total patient/family hospice care under the supervision of the Director and with the interdisciplinary team
Collaborates with the interdisciplinary team in the development, review and revision of the clinical component of patient/family plan of care
Ensures continuity of care between patient/family, team members, ancillary providers, long term and inpatient care facilities, and the attending physician
Communicates patient/family/caregiver needs, ongoing nursing assessment, interventions, goals and outcomes through the interdisciplinary process
Provides and documents continuing education of the hospice concept of care to patients and their families/caregivers
Supervises the delivery of patient care provided by Hospice Aides, Licensed Practical Nurses
Facilitates the delivery of hospice services to patients residing in long term care facilities
Participates in on-call rotation for delivery of care after office hours, on weekends, and holidays
