Job Description:

This is a PRN position and will primarily service patients in the South Metro Denver area. CHI Health at Home is a ministry of humankindness, extending the healing presence of a vast healthcare network into the most personal of settings. Job Responsibility:

  • Apply expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care
  • Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being
  • Work seamlessly with a multidisciplinary team to ensure holistic care
  • Serve as a vital resource for patients and their families, providing crucial teaching and emotional support
  • Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides
  • Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation

Requirements:

  • Graduate of an accredited nursing program with an active RN license in the state served
  • Minimum of 1 year of nursing experience
  • Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care
  • Current CPR/BLS certification
  • Valid driver’s license and reliable transportation

Nice to have:

Home Health experience is a significant plus
