Job Responsibility:
Assess and evaluate patient needs to develop and implement individualized care plans
Coordinate and oversee the delivery of patient care in a home setting
Serve as a primary contact for patients and their families regarding care and treatment
Collaborate with healthcare teams to ensure comprehensive patient care
Monitor and document patient progress and outcomes in compliance with policies
Conduct patient education regarding their health, care plans, and disease management
Manage clinical documentation and ensure adherence to regulatory standards
Requirements:
Current RN license in the state of practice
Minimum of 2 years nursing experience, preferably in home health or community health
Strong assessment, critical thinking, and clinical skills
Excellent communication skills both verbal and written
Ability to work independently and as part of a multidisciplinary team
Familiarity with electronic medical records (EMR) systems
Compassionate and patient-centered approach to care
