Job Responsibility

Have the knowledge of community health concepts, principles of ambulatory clinical nursing care, case management and care coordination

Utilizes best practice to maximize patient engagement and patient self-management efforts
Promote the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act
