RN Care Coordinator II - Infectious Disease

The Infectious Disease division cares for patients with general and complex diagnoses and symptoms with acute and long term diseases. The Infectious Disease Care Coordinator, in collaboration with the Infectious Disease specialty medical providers and medical assistant, performs case management and direct patient care to assigned patients. The coordinator role in Infectious Disease, has a blend of hand on procedures, complex patient care coordination, education on disease management, triage, and in clinic support. We are looking for someone that is autonomous, collaborative, supportive of teams, educator, customer focused, quick learner, and solution driven. The coordinator would be joining a dynamic division with a collaborative partnership with Providers and ancillary support staff as well as an engaged and supportive team.
