Minimum of 1 year's experience working in a GP surgery
Confident working autonomously within a primary care setting
Strong clinical knowledge with particular confidence in medication reviews and managing long-term conditions

Job Responsibility

Carrying out structured and clinical medication reviews (face-to-face and remote)
Supporting safe and effective prescribing within the practice
Managing long-term conditions such as asthma, hypertension, and other LTCs
Undertaking patient consultations (face-to-face and telephone)
Working alongside the wider practice team to improve patient outcomes and reduce GP workload
