Our Health Reablement Teams and urgent care team are seeking enthusiastic and dedicated Rehabilitation Support Workers with a sound knowledge and experience of working with complex, frail and older people. CityCares Health Reablement and urgent care services deal with a wide range of cases, providing care and rehabilitation for people presenting with complex pathologies, as well as challenging medical and social circumstances. We are seeking staff with a wide range of holistic health and social care skills and experiences to complement our service. Visiting patients within their own homes, the teams are dedicated to preventing unnecessary hospital admissions and promoting independence and quality of life. You will be working closely with Occupational therapists, Physiotherapists and Nurses as well as GPs, District Nurses, Community Matrons, the acute trust and a range of Health and Social Care Professionals. The aim of our service is to prevent unnecessary hospital admissions and to facilitate timely and safe discharges from the acute setting. We promote independence and with intensive rehabilitation and assessment, we strive to help our service users to achieve their goals, however big or small. Rehabilitation Support Workers practise under the guidance and supervision of the Clinical team.
Job Responsibility:
Support patients/clients to improve or maintain their level of function and quality of life and support care in an appropriate setting in partnership with multi-disciplinary team (MDT) colleagues
Support, encourage and supervise care plans put in place by a clinician to support a patient with their daily activities of living to get them back to their base line
Support patients with exercises outlined by the clinical team, feeding back any areas for improvement or decreasing in mobility
Supporting a patient with medications
Participate actively in the services training strategy
To work with the at home clinical team to implement rehabilitation and ongoing care programmes
To carry out goal focused programmes designed by the clinical team
To provide any essential care to maintain a persons health and wellbeing and feedback to the team regarding ongoing care needs
To undertake rehabilitation and evaluation of clients care programmes
To use skills in motivation, negotiation and facilitation to achieve desired outcomes
To occasionally provide and fit equipment following an emergency or crisis intervention
To educate clients and their carers in the safe and correct use of equipment
To ensure effective communication and exchange of information with clients/carers
To liaise with multi-professional teams and other agencies
To adapt the approach used to take account of clients/carers individual circumstances
To feedback, monitor and update on the progress and needs of clients/carers
To write and maintain accurate and comprehensive records
To comply with health and safety requirements
To undertake administrative, clerical and housekeeping duties
To ensure effective use of time and organisational skills
To maintain appropriate statistical returns
To assist in the organisation of home assessment visits
To actively contribute to the promotion and marketing of the service
To plan and prioritise own workload
To assist with induction of new staff
To attend organised training sessions and complete all relevant competences
To be involved in appraisal systems, clinical supervision
To keep an up-to-date CPD portfolio
To assist in the training and development of others
To keep up to date in basic IT skills
To participate in appropriate in-service training
Requirements:
Basic understanding of community care objectives and practices
Awareness of rehabilitation and promoting independence

Ability to prioritise and plan workload

Completion of a basic education
Desire to, or working towards NVQ level 2 in Health and Social Care
All appointments are subject to satisfactory medical clearance and the ability to cope with the demands of the role
This post requires shift working between 7am -10pm 5 days over 7 including weekend and bank holidays
Full UK driving licence access to own car and the ability to travel between locations is essential to this role
Experience of working with older people. This could include substantial voluntary work or work in a related field e.g. housing
Experience of record keeping and I.T.
