Job Responsibility:
Responsible for coordinating with the interdisciplinary treatment team to evaluate and screen patients for admissions to the inpatient rehabilitation unit and Rehabilitation Service
Conducts Pre-Admission Screening and facilitates the delivery of services, evaluates effectiveness, tracks outcomes and functions as the patient advocate to identify and communicate health care needs across the healthcare continuum
Develops, implements, and evaluates patients for comprehensive therapy program for all medically referred patients
Assists with educational, administrative, managerial, and quality control duties as assigned
Requirements:
BSN preferred
A minimum of 2 years nursing experience preferred
Work experience in rehabilitation case management, utilization review or hospital quality assurance is preferred
Bilingual in Spanish/English is preferred
Current registration with the Texas State Board of Nurse Examiners is required
Case Management Certification (CCM or ANCC) is preferred
Current American Heart Association, Basic Cardiac Life Support and Health Care Provider card required
