Job Responsibility:

  • Responsible for coordinating with the interdisciplinary treatment team to evaluate and screen patients for admissions to the inpatient rehabilitation unit and Rehabilitation Service
  • Conducts Pre-Admission Screening and facilitates the delivery of services, evaluates effectiveness, tracks outcomes and functions as the patient advocate to identify and communicate health care needs across the healthcare continuum
  • Develops, implements, and evaluates patients for comprehensive therapy program for all medically referred patients
  • Assists with educational, administrative, managerial, and quality control duties as assigned

Requirements:

  • BSN preferred
  • A minimum of 2 years nursing experience preferred
  • Work experience in rehabilitation case management, utilization review or hospital quality assurance is preferred
  • Bilingual in Spanish/English is preferred
  • Current registration with the Texas State Board of Nurse Examiners is required
  • Case Management Certification (CCM or ANCC) is preferred
  • Current American Heart Association, Basic Cardiac Life Support and Health Care Provider card required
