Job Description:

Proline Care are seeking compassionate and dedicated Care Workers to join our rehabilitation team. As a Rehabilitation Care Worker, you will provide essential support and assistance to individuals in their own homes on discharge from hospital with the aim of promoting independence and re-enablement. This is a rewarding opportunity to make a positive impact on the lives of others.

Job Responsibility:

  • Following personalised care plans to maximise independence
  • Assist individuals with daily activities such as personal hygiene, dressing, and meal preparation
  • Administer medication as directed by healthcare professionals
  • Provide emotional support and companionship to individuals in your care
  • Assist with light housekeeping tasks, including cleaning and organizing living spaces
  • Help individuals with mobility issues, using proper lifting and transferring techniques
  • Communicate effectively with individuals, their families, and healthcare professionals
  • Utilize IT systems to accurately document care provided

Requirements:

  • Fluent in English, both written and verbal
  • Previous experience in a care home or home care setting is preferred
  • Basic IT skills for documentation purposes
  • Ability to follow care plans effectively
  • Excellent communication skills to interact with individuals, families, and healthcare professionals
  • Valid driver’s license and access to own vehicle
