Job Description:

Proline Care are seeking compassionate and dedicated Care Workers to join our rehabilitation team. As a Rehabilitation Care Worker, you will provide essential support and assistance to individuals in their own homes on discharge from hospital with the aim of promoting independence and re-enablement. This is a rewarding opportunity to make a positive impact on the lives of others.

Job Responsibility:

Following personalised care plans to maximise independence
Assist individuals with daily activities such as personal hygiene, dressing, and meal preparation
Administer medication as directed by healthcare professionals
Provide emotional support and companionship to individuals in your care
Assist with light housekeeping tasks, including cleaning and organizing living spaces
Help individuals with mobility issues, using proper lifting and transferring techniques
Communicate effectively with individuals, their families, and healthcare professionals
Utilize IT systems to accurately document care provided

Requirements:

Fluent in English, both written and verbal
Previous experience in a care home or home care setting is preferred
Basic IT skills for documentation purposes
Ability to follow care plans effectively
Excellent communication skills to interact with individuals, families, and healthcare professionals
Valid driver’s license and access to own vehicle
