The Registered Practical Nurse (RPN) is responsible for assessing, planning, and providing patient care interventions in accordance with the standards of practice set by the College of Nurses of Ontario and the goals of the Hospital to Home (H2H) program. The RPN delivers clinical care to patients transitioning from hospital to home, including Emergency Department patients, inpatients designated as Alternate Level of Care (ALC), or those at risk of becoming ALC, who require professional services related to optimization, restorative, reactivation, or rehabilitation care. Working within a standardized and integrated care model, the RPN supports timely hospital discharges, continuity of care, and reduced ALC pressures while involving patients, families, and significant others through the principles of health promotion and quality improvement.
Job Responsibility
Provides patient-centered nursing care in accordance with the standards of the College of Nurses of Ontario and organizational policies
Assesses, plans, implements, and evaluates care for assigned patients within scope of practice
Collaborates with the Patient Care Manager, interdisciplinary teams, and community partners to support patient care and program goals
Supports continuity of care and safe transitions from hospital to home, including discharge planning and arranging community supports
Performs nursing procedures and clinical duties while maintaining patient safety and quality care standards
Acts as a patient advocate and develops supportive relationships with patients and families
Communicates effectively through verbal, written, and electronic documentation
Participates in patient care conferences, care planning, and interdisciplinary team meetings
Maintains patient and staff confidentiality and adheres to professional standards and organizational procedures
Supports quality improvement initiatives, incident reporting, and best practice standards
Assists with orientation, education, and mentorship of staff and students
Demonstrates strong clinical judgment, critical thinking, care coordination, and rehabilitation/reactivation knowledge.
Requirements
Graduate of an R.P.N. Certificate Program
CPR and Basic Life Support (BLS) certification
Current Certificate of Competence from the College of Nurses of Ontario as Registered Practical Nurse
Must have valid license and provide a good driving abstract
