Job Description:

Community Registered Nurse role delivering in-home clinical care to help older Australians stay safe, well and independent. A part-time, hybrid role with flexible hours, no night shifts or weekends, focusing on person-centred care in the Central Coast region.

Job Responsibility:

Deliver high-quality clinical nursing care in line with individual client needs
complete assessments, develop care plans, and provide wound care, catheter management, stoma care, diabetes management and chronic disease management
collaborate with GPs and allied health professionals to ensure holistic care
empower clients through education and promote health and wellbeing

Requirements:

Division One RN, registered with APHRA
minimum 1 year of experience in community, aged care, or acute settings
proficient use of technology, including mobile platforms and provision of telehealth services
police check or NDIS workers check
Working with Children check
CPR
valid driver's licence and access to a reliable, insured vehicle
passion for delivering person-centred care in the community
