Job Description:

Registered nurses at Phoenix Home Care & Hospice focus their energy on what’s most important to their patients. You will provide one-on-one care to our patients, treating them with the compassion they deserve. As an RN on our team, you can create a flexible schedule to fit YOUR needs.

Job Responsibility:

Assisting in the development, reviewing, and revisions of a plan of care
Providing one-on-one nursing care in accordance with the physician’s plan of treatment and individual plan of care, as authorized by the client’s payer source
Observing, recording and reporting reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician
Providing ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment
Participating in education programs, quality assessment activities, nursing meetings and other staff meetings as required

Requirements:

One year of experience in Home Health or Hospice preferred
Active nursing license in good standing
Driver’s license and vehicle with current auto insurance
Positive, professional attitude
