Baptist Health is looking to add a Registered Nurse (RN) to our Hospital at Home team. Baptist Health is opening an innovative Hospital at Home program that allows patients to complete their hospital stay at home. Patients that will be part of this program are provided with remote patient monitoring (RPM), technology kits, and communication devices. A 24-hour command center staffed by nurses monitors patients virtually, and contracted personnel provide in-home care when required. Job Responsibility

  • Provide virtual patient care for individuals located in Downtown and across the Baptist Health system
  • Providing comprehensive patient care through all phases of the nursing process — assessment, planning, implementation, and evaluation
  • Organizing and supervising care provided by supportive personnel while coordinating care with ancillary services
  • Educating patients, significant others, and families on disease processes, treatments, and care plans
  • Serving as a patient advocate to ensure high-quality, compassionate care
  • Administering treatments and medications as prescribed and monitoring patient responses
  • Managing care for patients with a variety of medical, surgical, and chronic conditions, as well as minor injuries or illnesses
  • Promoting Baptist Health’s mission of delivering excellence in care through empathy, safety, and teamwork

Requirements

  • Associate's Degree - Nursing
  • 2 years Acute Care Nursing Experience Required
  • Basic Life Support (BLS) Required
  • Licensed Registered Nurse Required
