Under the direction of the Clinical Manager – works weekend shifts assisting team leader and scheduling. Completes triage of incoming calls from patients/families/caregivers and assists with prioritizing clinical visits.
Job Responsibility:

Completes triage of incoming calls from patients/families/caregivers and assists with prioritizing clinical visits
Scheduling: Schedules and prioritizes eve /weekend/holiday nursing visits according to patient acuity and the most appropriate nurses on each shift
Supervision: Assists in providing direct clinical supervision to weekend and per visit nurses
Intake: Receives new patient referral information, on the eve/ weekend/holiday shift, completing the referral through appropriate inquiry and identification of patient needs
Communication: Communicates and collaborates with other members of the healthcare provider team, as needed on the weekend/holiday shift, to obtain and clarify orders or to discuss changes in the patient condition
Direct Care: Updates plan of care with any change in patient status
Performs telephonic comprehensive physical and psychosocial assessments
Professional Development: Maintains current knowledge of nursing practice by attendance at in-service and participation in seminars and courses to increase knowledge of community health nursing
Technology: Embraces technological solutions to work processes and practices
Safety: Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment
Educator: Employs expertise as a role model, preceptor and/or coach
Advanced Clinical Skills: Participates in educational opportunities to develop and maintain specialized clinical skills
Requirements:
Graduate of an NLN approved school of nursing
Equivalent to a Bachelor of Science Degree in Nursing in an NLN–approved school of nursing preferred
Requires two (2) years of acute care experience
Two years of community health nursing experience preferred
Read, write and speak English
Current valid Driver's License
Excellent assessment skills
Homecare experience a plus
Required 1 weekend per month, Occasional Eve and Holiday Commitment
RN-Registered Nurse - Board of Registration in Nursing (Massachusetts)
