This Full-time 10:45 PM - 7:15AM position is eligible for benefits and will be scheduled to work 80 hours per 2-week pay period with every-other weekend scheduled. The Skilled Nursing Facility (SNF) at Champlain Valley Physicians Hospital accommodates 34 residents, serving a diverse range of patients, spanning from middle-aged adults to geriatric residents. It maintains a balance between long-term care and subacute care. Short-stay patients typically stay for 1 to 6 weeks on average, receiving specialized care for conditions such as sepsis, post-orthopedic surgery recovery, and wound care. For the subacute population, it provides a spectrum of skilled nursing and therapy services, including wound care with advanced techniques like wound vacuums, antibiotic therapy, and comprehensive physical, occupational, and speech therapy.
Job Responsibility:
Provide compassionate, individualized care
Support residents in achieving their highest level of health and independence
Work with a diverse range of patients from middle-aged adults to geriatric residents
Contribute to discharge planning processes
Enhance medical-surgical skills
Form meaningful relationships with patients
Requirements:
Current RN licensure or compact licensure recognized by the State of New York required
Appropriate experience in specific clinical area
BLS or ACLS required with ACLS certification must be achieved within 6 months of hire
Nice to have:
