Current, unrestricted Oregon RN license
Previous experience in home health, hospice, or acute care preferred
Strong clinical judgment and assessment skills
Strong understanding of In-home care and home health regulations
CPR certification and TB clearance required
Valid driver's license, insurance, and reliable transportation
Strong communication, organization, and clinical judgment skills

Job Responsibility

Perform in-home client assessments and re-assessments
Provide nursing services within RN scope of practice
Support development and updates of client care plans
Monitor client status and communicate changes to the clinical team
Document all visits accurately and in a timely manner
Collaborate with the Clinical Manager and care team to ensure quality care
Provide patient education and support to clients and families
