Job Responsibility

  • Conduct comprehensive in-home nursing assessments and ongoing evaluations of participant status
  • Perform skilled nursing visits including medication management, wound care, injections, phlebotomy, and chronic disease monitoring
  • Assess changes in condition and communicate promptly with providers and the interdisciplinary team (IDT)
  • Develop, implement, and monitor individualized care plans in collaboration with the IDT
  • Provide participant and caregiver education on disease management, medications, safety, and prevention strategies
  • Coordinate care transitions following hospitalizations, ER visits, or facility stays
  • Support participants residing in ALF or other community settings as needed
  • Identify safety concerns in the home and collaborate with PT/OT and social work to address environmental risks
  • Participate in interdisciplinary team meetings and contribute to holistic care planning
  • Document all nursing care accurately and timely in the electronic medical record
  • Respond to urgent clinical needs and participate in on-call rotation as assigned
  • Monitor medication adherence and assist with medication setup or reconciliation
  • Collaborate with external providers, pharmacies, home health agencies, and specialty services
  • Perform other related duties as assigned.

Requirements

  • Current Registered Nurse (RN) licensure in the Commonwealth of Virginia
  • Current BLS/CPR certification
  • Minimum of one year of experience working with the frail or elderly population
  • Strong clinical assessment and critical thinking skills
  • Medical clearance for communicable diseases and up-to-date immunizations prior to participant contact
  • Valid driver’s license, proof of insurance, and reliable transportation (required due to field-based nature of the role)
  • Ability to work independently while maintaining strong communication with a collaborative team.

Nice to have

Experience with home health, geriatrics, hospice, or community-based care preferred.
