Job Responsibility:

  • Assess patient and family needs and develop personalized care plans
  • Deliver skilled

  • Administer medications and coordinate care across the healthcare team

Requirements:

  • Current RN license in Virginia (multi-state or single-state)
  • CPR certification
  • at least two years of nursing experience (hospice or home health preferred)
