We’re looking for a dedicated Registered Nurse Assessment (RN Assessment) to support patients and families as they enter hospice care. This role is essential in ensuring a smooth and compassionate start to the hospice journey. You’ll complete initial assessments, develop individualized care plans, and provide education and support during the admission process. You’ll collaborate closely with our interdisciplinary team to make sure patients receive care that aligns with their wishes, goals, and needs.
Job Responsibility:
Complete intake assessments for referred patients and coordinate follow-up
Assess medical, physical, emotional, and social needs and form nursing diagnoses
Develop individualized care plans in collaboration with the interdisciplinary team
Educate patients and families on hospice services, symptom management, and care expectations
Provide emotional support
Advocate for patients
Implement nursing interventions focused on comfort, pain management, and symptom relief
Provide hands-on teaching to families, caregivers, and home health aides
Follow universal precautions and isolation techniques
Maintain accurate, timely clinical documentation
Respond to questions from individuals contacting hospice and serve as a referral resource
Participate in training and orientation of new team members
Educate staff in other care settings on hospice philosophy
Work collaboratively within the interdisciplinary team
Maintain patient confidentiality and autonomy
Seek ongoing opportunities for professional growth
Recognize when family systems are not amenable to change and adjust care plans
Act independently and responsibly
Share weekend and holiday rotation with the Intake Team
Maintain active driver's license and reliable transportation
Other duties as assigned
Requirements:
Active RN license in the state of Virginia
At least one year of medical-surgical or oncology experience
Minimum of six months hospice home care experience preferred
Proficiency with EMR systems, including Epic
Strong communication and assessment skills
1 year minimum experience
Nice to have:
BSN preferred
Minimum of six months hospice home care experience preferred
