Job Description:

After Hours Registered Nurse (RN) to provide critical support to patients and families across the Blue Ridge Care territory during evenings, nights, weekends, and holidays. This role is ideal for a compassionate nurse who thrives in independent decision-making and is dedicated to delivering quality hospice care outside of traditional hours.

Job Responsibility:

  • Answer emergent patient calls and conduct visits as needed to address immediate concerns
  • Provide professional nursing care consistent with patient care goals and the physician’s plan of care
  • Assess and evaluate patient status
  • if working with LPN support, provide guidance and oversight
  • Develop and update care plans with goals based on nursing diagnoses
  • Administer medications and treatments as prescribed, including preventive and rehabilitative care
  • Counsel patients and families on health care needs, offering education and guidance
  • Teach self-care techniques, including medication administration, diet, and symptom management
  • Maintain accurate and timely clinical documentation of patient condition and care
  • Communicate with physicians regarding patient changes and obtain/document orders
  • Ensure safe patient environments and arrange necessary equipment/services
  • Conduct initial assessments and collaborate with patients, families, and the care team to ensure seamless care transitions
  • Engage in continuing education and quality improvement activities
  • Support initiatives like peer chart reviews, medication reconciliation, and tuck-in programs.

Requirements:

  • Current RN license in the Commonwealth of Virginia
  • Graduate of an accredited nursing program
  • 2+ years of experience in hospice, home health, or ICU/CCU (preferred)
  • CHPN certification (preferred)
  • Knowledge of Medicare/Medicaid reimbursement regulations
  • Current CPR certification
  • Strong communication and interpersonal skills
  • Proficiency in electronic medical records and computer skills
  • Ability to travel throughout the Blue Ridge Care territory
  • Weekend availability only will be considered.

Nice to have:

  • BSN preferred
  • 2+ years of experience in hospice, home health, or ICU/CCU (preferred)
  • CHPN certification (preferred)
