Graduate of an accredited nursing program
2+ years of experience in hospice, home health, or ICU/CCU (preferred)
CHPN certification (preferred)
Knowledge of Medicare/Medicaid reimbursement regulations
Current CPR certification
Strong communication and interpersonal skills
Proficiency in electronic medical records and computer skills
Ability to travel throughout the Blue Ridge Care territory
Weekend availability only will be considered

Job Responsibility

Answer emergent patient calls and conduct visits as needed to address immediate concerns
Provide professional nursing care consistent with patient care goals and the physician’s plan of care
Assess and evaluate patient status
if working with LPN support, provide guidance and oversight
Develop and update care plans with goals based on nursing diagnoses
Administer medications and treatments as prescribed, including preventive and rehabilitative care
Counsel patients and families on health care needs, offering education and guidance
Teach self-care techniques, including medication administration, diet, and symptom management
Maintain accurate and timely clinical documentation of patient condition and care
Communicate with physicians regarding patient changes and obtain/document orders

Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician
Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions
Documents medication regimen and updates medication profile according to established procedures
