Licensed as a Registered Nurse in the Commonwealth of Massachusetts
Baccalaureate degree in nursing required within 4 years of hire

Job Responsibility

Participates in day-to-day operations of the program including patient education and call triage
Participates in program evaluation and development of strategic goals

Assumes nursing care responsibilities for a defined group of patients within the program, including but not limited to assessment, education, coordination, and documentation

Supports a smooth transition of patients from active treatment into survivorship, chronic cancer management, or end-of-life care
Assists patients with cancer with issues related to treatment goals, advanced directives, palliative care, and end-of-life concerns
Acts as a liaison between the patients, families, and caregivers and the providers to optimize outcomes
Advocates for patients to promote patient centered care

Registered Nurse license issued by the state in which the team member practices
Bachelor's degree in nursing

Supports the patient and family by providing education, addressing psychosocial needs, advocating, providing continuity of care and identifying barriers to care
Navigates patients and family members throughout the diagnosis, treatment and follow-up of the patient throughout the continuum of care
