The Integrated Care Homes Service is looking for Registered Nurses with community nursing experience, or a student nurse who has completed their management placement in the community, to join our developing team. As a Registered Nurse, you will provide nursing interventions to a wide range of patients, by in reaching into Nottingham City Care Homes to deliver NHS services. The services overarching aim is to ensure that care home residents and staff have an improved experience through high quality care and access to familiar health professionals with the skills to meet their needs. A single point of access enables the care home staff to refer patients directly to the service enabling the prioritisation and allocation of referrals. The successful candidates will require up to date clinical knowledge and skills and will be required to work autonomously, using initiative to deliver nursing care within a supportive team environment. They will be working with a diverse population providing support and nursing care to people who have complex health problems, with the aim to maintain and maximise their independence and prevent avoidable hospital admissions. You will be supported to learn and develop the skills required for this role.
Job Responsibility:
Provide community nursing care to a wide variety of patients in residential homes across Nottingham City
Work within the wider integrated care homes service providing holistic care to patients
Provide holistic and individualised care to patients and carers as delegated by the Team Leader or deputy
Regularly take responsibility for care and workload management in the absence of the Team Leader or deputy
Provide nursing care across a diverse population for patients who are living within a care home
Participate actively in the services training strategy
Work across the Nottingham City Integrated Care Homes and Community Nursing Teams
Supervise and support student nurses on placements
Supervise unregistered nurses who have a delegated caseload
Contribute to the public health agenda
Plan, deliver and evaluate programmes of care
Implement the provision of evidence-based/ best practice
Actively encourages patient and carer feedback
Maintain a high standard of care
Contribute to the continual improvement of the service
Maintain a professional image
Contribute to the Clinical Governance agenda and applies risk management systems
Participate in relevant research and audit activity
Regularly takes responsibility for the appropriate delegation of nursing care

In the absence of District Nurse Team Leader or deputy takes responsibility for the needs of patients and carers on the community nursing caseload
Requirements:
RN qualification on part 1 or part 2 of the register
Post registration experience or completion of a community management placement
Up to date clinical knowledge, including wound care, management of continence and palliative care
Understanding of equality and diversity
