Registered nurse - home care

• Provide skilled nursing services to home care clients in accordance with individualized care plans, physician orders, and regulatory guidelines
• Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations
• Administer medications, treatments, wound care, and other clinical procedures as appropriate for the patient’s needs
• Educate patients and caregivers on disease processes, treatment plans, medications, and home safety
• Communicate changes in patient condition to the physician and clinical team promptly
• Collaborate with other members of the multidisciplinary team
• Maintain accurate, timely, and compliant documentation in the electronic medical record
• Adhere to infection control standards and ensure patient safety in all care settings
• Provide compassionate, patient‑centered care while promoting independence and optimal health outcomes

• Minimum of one (1) years recent experience as a professional nurse in a clinical setting. Home Health experience preferred
• Must complete Point of Care Testing training as part of Department Orientation
• Valid DL With No More Than 3 Moving Violations
• Proof Of Current/Valid Car Insurance
• RN - Registered Nurse
• BLS - Basic Life Saving certification (American Heart Association)

Home Health experience preferred
