The Home Care Nurse is a licensed professional responsible for delivering skilled nursing care to clients in their homes. This role ensures high‑quality clinical care through thorough assessments, timely interventions, and compassionate support. The nurse collaborates closely with the home health multidisciplinary team, following physician orders and working under the direction and supervision of the Clinical Supervisor.
Job Responsibility:
Provide skilled nursing services to home care clients in accordance with individualized care plans, physician orders, and regulatory guidelines
Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations
Administer medications, treatments, wound care, and other clinical procedures as appropriate for the patient’s needs
Educate patients and caregivers on disease processes, treatment plans, medications, and home safety
Communicate changes in patient condition to the physician and clinical team promptly
Collaborate with other members of the multidisciplinary team
Maintain accurate, timely, and compliant documentation in the electronic medical record
Adhere to infection control standards and ensure patient safety in all care settings
Provide compassionate, patient‑centered care while promoting independence and optimal health outcomes
Requirements:
Minimum of one (1) years recent experience as a professional nurse in a clinical setting. Home Health experience preferred
Must complete Point of Care Testing training as part of Department Orientation
Valid DL With No More Than 3 Moving Violations
Proof Of Current/Valid Car Insurance
RN - Registered Nurse
BLS - Basic Life Saving certification (American Heart Association)
