Job Description:

The RN Field Assessor conducts in-person assessments with members to evaluate their medical needs and support their overall wellness. This role helps members access essential services such as adult and pediatric medical daycare, personal care assistance, nursing facility custodial care, the Personal Preference Program, and MLTSS program enrollment. Assessors are expected to complete 10–15 assessments per week, depending on demand. Each assessment incorporates information from multiple sources to evaluate all health conditions, including co-morbidities and complex diagnoses that affect functionality.

Job Responsibility:

  • Conducts in-person assessments with members to evaluate their medical needs and support their overall wellness
  • Helps members access essential services such as adult and pediatric medical daycare, personal care assistance, nursing facility custodial care, the Personal Preference Program, and MLTSS program enrollment
  • Complete 10–15 assessments per week
  • Evaluate all health conditions, including co-morbidities and complex diagnoses that affect functionality
  • Evaluations also consider the member's work capacity and any related limitations
  • Using a holistic approach, determines the need for referrals to clinical resources
  • Collaborates with supervisors and team members to overcome barriers to care
  • Follows established case management protocols in alignment with regulatory and company policies
  • Serve as a mentor to new team members

Requirements:

  • Must possess reliable transportation and be willing and able to travel up to 75-100% of the time within Somerset and Middlesex counties
  • Must be comfortable completing Field based assessments for all socioeconomic status and settings
  • Must hold an active unrestricted Registered Nurse license in the state of New Jersey
  • 2+ years clinical practice experience e.g., hospital setting, alternative care setting such as home health or ambulatory care required
  • Successful completion of the NJ Choice certification is a contingency of employment
  • Must be able to work Monday - Friday, 8:00am - 5:00pm EST
  • Minimum Associates Degree in Nursing required

Nice to have:

  • Assessment and Case management and/or discharge planning experience
  • Managed Care experience
  • Crisis intervention experience
  • NJ Choice certification
  • Clinical interviewing
  • Experience using personal computer, keyboard navigation, navigating multiple systems and applications
  • and using MS Office Suite applications (Teams, Outlook, Word, Excel, etc.)
  • Bachelors in Science and Nursing preferred
