Job Description

We are looking for a Registered Nurse to join our Hospice at Home Team! This is a Community role based from the Hospice in Cambridge, working in patients homes across Cambridgeshire. The Hospice at Home service cares for patients across Cambridgeshire who choose to remain in their own home in their last weeks of life. We are holistic in our approach and the care is tailored to each person’s specific needs.

Job Responsibility

  • Work directly and alone with patients and families in their own homes
  • provide professional, patient centred palliative care
  • ensure the highest possible standards of care are received by patients, their families and carers
  • lead, support and encourage the sharing of knowledge and skills within the team
  • provide practical and holistic care supporting patients and their families in their homes
  • work flexibly to meet patient needs across a wide geographical area

Requirements

  • NMC Registered Nurse (Adult)
  • post registration experience in palliative or end of life care
  • ability to work as part of a team
  • excellent communication skills
  • excellent leadership skills
  • valid driving license
  • access to a vehicle which can be used for work
  • subject to an Enhanced DBS check with Barred list checks

  • Bachelor of Science in Nursing (for RNs)
