Job Description:

Baptist HealthCase Manager is looking for a Registered Nurse, at Baptist Medical Jacksonville - Downtown campus. This is a Full Time Day shift opportunity working Monday - Friday in the In-Patient Case Management Department at Baptist Jacksonville - Downtown campus.

Job Responsibility:

  • Care coordination for hospitalized patients
  • High risk screenings and assessments
  • Coordinating post acute care services with proper documentation
  • Collaborating with multidisciplinary team members

Requirements:

  • Associate's Degree - Nursing
  • 1 year Case Management Experience
  • Cardiopulmonary Resuscitation Certification (CPR) Required
  • Licensed Registered Nurse Required

Nice to have:

Bachelor's Degree - Nursing
