Job Description:

As a Case Manager, you will play a critical role in coordinating care for hospitalized patients, ensuring smooth transitions through the continuum of care, and working closely with the multidisciplinary team to optimize patient outcomes.

Job Responsibility:

  • Perform high-risk screenings and comprehensive assessments
  • Coordinate care plans and post-acute services with accurate documentation
  • Facilitate communication among providers, patients, and families
  • Support safe and timely discharges through effective resource utilization

Requirements:

  • Associate's Degree - Nursing
  • 1 year Case Management Experience
  • Cardiopulmonary Resuscitation Certification (CPR) Required
  • Licensed Registered Nurse Required
