Job Description:

The Clinical Beds team provides holistic assessments to patients who require a period of rehabilitation and assessment prior going home after being discharged from hospital. We work closely as a multidisciplinary team alongside our partner services such as Adult social care and continuing health care to aid patients and families make long term decisions about their health and social care needs. This is an excellent opportunity to broaden your skill set in a holistic manner working with physiotherapists, occupational therapists and mental health nurses along with supporting junior colleagues such as student nurses, assistant practitioners and rehabilitation support workers.

Job Responsibility:

  • Work as an autonomous practitioner, undertaking comprehensive assessments to identify and meet the nursing needs for individuals under the care of the clinical beds team.
  • Carry out profession specific and holistic assessments, creating and evaluating care plans as required.
  • Plan and develop goal focus rehabilitation/reablement programmes for adults with a wide variety of complex physical and mental health needs.
  • Facilitate discharge into an appropriate setting.
  • Be responsible for continuing the ongoing development of high standards of clinical practice with evidence-based focus.
  • Work across different bases and community settings which may include a clients own home, community bed, nursing home or hospital.
  • Be responsible for a designated caseload of clients.
  • Participate actively in City Cares training requirements.
  • Supervise, educate and assess the competency of junior staff, support staff and students.
  • Support the Team Manager in implementing and evaluating targets and objectives.
  • Advise the Team Manager in relation to clinical needs of the client group.
  • Take delegated responsibility for management duties as required.
  • Ensure the health & safety of self and others.
  • Use information technology on a daily basis and participate in identified data collection and statistics.
  • Demonstrate flexibility, prioritise effectively and time manage self.
  • Participate in the Lone Worker Tracking procedure.
  • Act as duty clinician for the process of referrals.
  • Be professionally and legally responsible and accountable for all aspects of your work.
  • Gain valid informed consent and to identify through assessment clients who lack capacity.
  • Undertake advanced comprehensive assessment and evaluation of adults presenting with a wide variety of complex physical and mental health needs.
  • Be responsible for coordinating the safe and timely discharge process from community beds.
  • Risk assess, advise and where appropriate provide equipment including teaching clients / carers in its use.
  • Provide highly specialist advice, education and mentorship to the other staff and colleagues.
  • Be responsible for own continuing professional development.
  • Participate in the supervision framework as a supervisee and a supervisor.
  • Be a clinical educator for undergraduate students on university placements.
  • Undertake CityCares Holistic Worker Framework competencies.
  • Work to your Code of Professional Conduct.
  • Maintain a high standard of care to clients and carers including accurate and detailed documentation.
  • Facilitate and carry out 1:1 and group education and training to colleagues and junior staff.
  • Contribute to Clinical Governance agenda and apply Risk Management strategies.
  • Contribute to implementation of service development and change as appropriate.
  • Be actively involved in Clinical Governance, implement policies and proposing practice changes.
  • Comply with organisations Lone Working Policy and local team guidelines.
  • Be responsible for own time and caseload management.
  • Liaise effectively with other professionals and agencies to enhance rehabilitation.
  • Ensure the Health and Safety of staff and clients at all times.
  • Comply with and complete statutory and mandatory training.
  • Attend training events as appropriate to support your continuing professional development.
  • Maintain a continuing professional development portfolio identifying learning objectives.
  • Actively participate in your own supervision, annual performance review and personal development plan.

Requirements:

  • Registered Nurse
  • NMC register
  • Post registration mental health experience
  • Ability to be flexible over hours worked within contracted hours to meet the needs of the service
  • This post will require shift working 5 days over 7 including weekend and bank holidays
  • Full driving licence and the ability to travel between locations, access to own vehicle for work purposes with business insurance
  • Minimum of 2 years post graduate experience at Band 5
  • Relevant experience of rehabilitation with adults
  • Community experience
  • Experience of working in a multi-disciplinary team
  • Experience of supervising junior, unqualified staff and students
  • Knowledge and experience of wide range of approaches in the management of complex long term conditions.
  • Evidence of achievement of appropriate competencies for clinical grade.
