Job Description:

The Clinical Beds team provides holistic assessments to patients who require a period of rehabilitation and assessment prior going home after being discharged from hospital. We work closely as a multidisciplinary team alongside our partner services such as Adult social care and continuing health care to aid patients and families make long term decisions about their health and social care needs.

Job Responsibility:

  • Work as an autonomous practitioner, undertaking comprehensive assessments to identify and meet the nursing needs for individuals under the care of the clinical beds team
  • Carry out profession specific and holistic assessments, creating and evaluating care plans as required
  • Plan and develop goal focus rehabilitation/reablement programmes for adults with a wide variety of complex physical and mental health needs
  • Facilitate discharge into an appropriate setting
  • Support the Team Manager in implementing and evaluating targets and objectives
  • Advise the Team Manager in relation to clinical needs of the client group
  • Ensure the health & safety of self and others
  • Use information technology on a daily basis and participate in identified data collection and statistics
  • Act as duty clinician for the process of referrals
  • Be professionally and legally responsible and accountable for all aspects of your work
  • Undertake advanced comprehensive assessment and evaluation of adults presenting with complex needs
  • Be responsible for carrying a client caseload and provide highly specialist clinical reasoning
  • Be responsible for coordinating the safe and timely discharge process from community beds
  • Risk assess, advise and where appropriate provide equipment
  • Provide highly specialist advice, education and mentorship to other staff and colleagues
  • Be responsible for own continuing professional development
  • Participate in the supervision framework as a supervisee and a supervisor
  • Be a clinical educator for undergraduate students
  • Work to your Code of Professional Conduct
  • Contribute to Clinical Governance agenda and apply Risk Management strategies
  • Be actively involved in Clinical Governance, implement policies and proposing practice changes
  • Be responsible for own time and caseload management
  • Liaise effectively with other professionals and agencies
  • Comply with and complete statutory and mandatory training

Requirements:

  • Registered Nurse
  • NMC register
  • Post registration mental health experience
  • Ability to be flexible over hours worked within contracted hours to meet the needs of the service
  • Full driving licence and the ability to travel between locations, access to own vehicle for work purposes with business insurance
  • Minimum of 2 years post graduate experience at Band 5
  • Relevant experience of rehabilitation with adults
  • Community experience
  • Experience of working in a multi-disciplinary team
  • Experience of supervising junior, unqualified staff and students
  • Knowledge and experience of wide range of approaches in the management of complex long term conditions
  • Evidence of achievement of appropriate competencies for clinical grade
  • Models of supervision
  • Evidence of continuing professional development based on best practice
  • Awareness of current local and national developments in health and social care skills
  • Ability to carry out comprehensive assessments and the development and evaluation of treatment plans
  • Able to work independently recognising own limitations
  • Excellent communication skills
  • Ability to relate well to all groups of people and to create and maintain effective working relationships within the team and externally
  • Skills supervising junior staff and students
  • Ability to assess and manage risk
  • Prioritisation and time management skills
  • Ability to motivate self and others
