Job Description:

Invicta Health in partnership with Kent and Medway NHS and Social Care Partnership Trust (KMPT) are seeking to recruit Community Mental Health Practitioners to be part of Mental Health Together. The Registered Mental Health Nurse / Occupational Therapist will be expected to deliver stepped care by delivering a range of evidence-based interventions which will promote recovery, social inclusion and normalization for service users who suffer from mental health problems. They will work in partnership with the MDT, CRHT, Acute Inpatients Services, other community teams, Primary Care and the Voluntary Sector to provide support and treatment to service users and facilitate the safe management of their needs within a range of community settings.

Job Responsibility:

  • Foster recovery through empowerment of service users and carers using a strengths-based approach
  • Offer a range of evidence-based interventions and risk management approaches
  • Be proactive, innovative, and creative in delivering care
  • Engage service users and carers in relapse prevention and risk management/contingency plans
  • Liaise with inpatient units to support discharge
  • Deliver a flexible and responsive service
  • Work collaboratively with the multi-disciplinary team and other agencies
  • Deliver high quality care and provide solutions and signposting
  • Act as an educator for service users and carers
  • Respond swiftly to urgent referrals
  • Work closely with carers
  • Recognise racial, cultural, sexual preference and address health inequalities
  • Ensure record keeping/documentation reflects needs
  • Maximize the use of resources
  • Be responsible for assessing, planning, implementing, and evaluating care

  • Minimum of 2 years post registration experience
  • Previous experience of working in a community team
  • Registered Mental Health Nurse, or registered Occupational Therapist (holding diploma or degree qualification)
  • MELPS (Mentoring and Enabling in a clinical setting)
  • Current UK professional registration
  • Right to work in the UK (no sponsorship provided)
