Job Description:

Solaris HealthCare Daytona is now hiring. We are looking for caring and compassionate individuals to provide outstanding care to our residents each and every day. The primary purpose of your job position is to coordinate nutritional care of residents by completing nutritional assessments, developing and implementing care plans, and documenting dietary information about residents, in accordance with current federal, state, and local standards, guidelines and regulations, and Solaris HealthCare’s established policies and procedures.

Job Responsibility:

Coordinate nutritional care of residents
Complete nutritional assessments, MDS, CAA’s and care plans
Complete progress notes
Assess and complete required documentation on residents nutritionally at risk
Visit residents routinely to determine effectiveness of interventions and to update food preferences and evaluate food intake
Promote and assist in implementation of liberalized diets
Follow house supplement procedure recommending enhanced foods before supplements
Evaluate the effectiveness of supplements on a routine basis
Review weight records routinely and communicate variances to interdisciplinary team
Make enteral feeding recommendations
Evaluate and monitor TPN
Ensure timely follow-up on physician recommendations
Communicate resident nutrition and hydration relevant changes in condition during the 24-hour Clinical Report/Stand-Up Meeting
Use current standards of dietetic practice in the implementation of nutritional care
Serve as Dining Room Coordinator, Dining Room Monitor as required
Oversee and ensure accuracy of nutritional reviews done by the Dietetic Technician and the Food Service Director

Requirements:

Registration with the Academy of Nutrition and Dietetics
Licensed by the state where he/she practices, if appropriate
At least one year experience in long term care, preferred
