Sapphire Nursing and Rehab at Goshen, a long term skilled nursing facility located in Orange County, New York is seeking a Registered Dietitian to join its team of exceptional care professionals. The Registered Dietitian will be responsible for assessing the nutritional status of patients and recommending the appropriate nutrition therapy as it pertains to the patients’ nutritional needs. The Registered Dietitian will also be responsible in assisting the dietary staff in creating high quality meals and the delivering of a meaningful dining experience for clients in a long tern skilled nursing facility.
Job Responsibility
Interview patient and/or family for nutritional history, obtains patient food preferences, assesses patients, and documents in patients’ medical record
Conduct nutritional assessment, develop and implement patient specific care plans, and assist in recommending nutrition care plans to medical staff for review and consideration
Assist in recommending therapeutic diet modifications and/or supplemental feedings to physicians for patients, and ensure accuracy of diet order received by checking against physician’s order
Calculate nutrient content of tube feeding and dietary intakes as ordered by physicians
Provide nutritional guidance and counseling to patients and their families
Assist with changes to patient’s menu, including patient food preferences, and the development of standardized recipes, as well as assist in overseeing food service workers in proper food handing and portion control
Prepare, present, and conduct individual and group education classes, as needed
Develop and utilize effective lines of communication regarding nutrition information within the facility by coordinating with an interdisciplinary team on a daily basis to provide quality care to residents
Requirements
Bachelor's Degree required, or equivalent combination of education and related experience
Current Registered Dietician Certification with the NYS Department of Education
Nice to have
Computer literacy and proficiency with EMR software and computer-based menu systems
Knowledge of applicable state and federal guidelines
