Job Description:

A leading live-in care provider is seeking an organised, people-focused Care Consultant based in Haywards Heath to support clients and carers across Sussex, with some travel into neighbouring areas such as, Surrey, and Kent. This role is predominantly field-based with flexibility to manage your own diary. You will oversee a manageable caseload of 12–14 clients, ensuring high-quality care delivery, regular client visits, and strong support for carers. You will work from home when completing documentation and travel within your region for assessments and visits.

Job Responsibility:

  • Manage a small portfolio of live-in care clients in the Sussex region
  • Complete client assessments, reviews, and risk assessments
  • Visit each client at least once per month
  • Support carers with personal care, complex needs and end-of-life situations when required
  • Maintain accurate documentation, care plans, and reports
  • Build strong relationships with clients, families, and carers
  • Respond to urgent changes in care needs and resolve concerns
  • Work independently while staying connected to a supportive care team
  • Attend team meetings and training as required.

Requirements:

  • A background in care or senior-level care experience (Senior Carer, Team Leader, Supervisor, Coordinator with face-to-face duties, etc.)
  • Experience working directly with clients or carers
  • Confidence with care plans, risk assessments, and documentation
  • Ability to support carers with personal care and end-of-life care
  • Highly organised with excellent time management
  • Comfortable working independently and managing your own diary
  • Adaptable, proactive, and confident handling urgent situations
  • Full UK driving licence and your own vehicle
  • Willingness to travel up to 1.5 hours from Haywards Heath.
