We’re seeking a dedicated professional to serve the Anson County community through a dynamic, split-location role. This position includes three in-person days each week—divided between the Anson Emergency Department and a local dental office—focused on direct patient and provider engagement. The remaining two days will be spent connecting with community organizations and patients to strengthen local health initiatives and outreach. M-F, 8-5. Occasional nights and weekends for events and scheduled patient call backs.
Job Responsibility:
Manage the intake of social needs referrals and requests for ASN CHW team services
Establish referral communication strategies for ASN team members
Assist with internal and community-based organization outreach and relationship building work
Provide training on the Find Help platform
Assist with organizing Find Help data and updating user groups
Help with Find Help technical management for listed ASN team programs
Manage the CRH group email and respond to inquiries from internal teammates and external community partners
Communicate with Community Health Worker team members to facilitate client care
Identify resources for clients to overcome barriers to care and facilitates access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services
Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance
Works to reduce cultural and socio-economic barriers between community members and institutions
Requirements:
High School Diploma or GED required
Bachelor's degree in a relevant public health or human service field preferred
Strong interpersonal skills, good communication and literacy skills, and demonstrates the ability to develop rapport with community health workers and care teams required
Strong understanding of cultural competency with the target population
Bilingual (English/Spanish) preferred
Computer literacy desirable
