Job Responsibility

Works with the patient and treatment team to develop substance abuse treatment goals and interventions, based on client's substance abuse/mental health history and current needs
Provides support, resources, education, and both internal and external referrals for services
Oversees client activities to ensure compliance with program activities and agency policies. Provides guidance as needed
Facilitates psychoeducational client groups and group activities
Provides crisis intervention and able to follow specific procedures in emergency situations
Attends and participates in all meetings and maintains audit-ready patient records and program-specific forms in a timely manner, as required
Completes individually assigned program-specific duties and/or responsibilities as assigned
