Job Description:

The Mental Health and Wellbeing Partnership, is a brand-new partnership of voluntary organisations in BaNES, Swindon and Wiltshire that includes Second Step, Alabaré, Missing Link and Nilaari. Our complimentary skills, knowledge, experience and community connections will ensure the development and delivery of excellent, accessible, person centred and integrated mental health services which are best placed to serve the diverse needs of communities in B&NES Swindon and Wiltshire. Missing Link provides practical advice and support to individuals to allow them to maintain a rich and meaningful life. The aim is to deliver recovery and wellbeing opportunities for people with mental health and other support needs to achieve their hopes and ambitions. As a Female* Recovery Navigator, you will work with colleagues from primary and secondary mental health services, social care, and other statutory and voluntary sector agencies to deliver community-based services. You will support the development of new ways of integrated working and play a key role in ensuring wrap-around support for service users. You will work within a recovery model to achieve a service user’s identified goals, in a responsive, trauma-informed, person-centred way. You will support service users to build confidence, and maintain hop and optimism for the individual with high aspirations for recovery.

Job Responsibility:

  • Work as part of an integrative partnership to provide a joined-up offer to individuals accessing the service
  • Liaise with colleagues and build a strong working partnership with other agencies such as GPs, Health and Social Care, other Voluntary and Community Services to coordinate referrals, to take initiative to share learning new ideas and trends, and deliver the service user’s support package
  • Attend and contribute in a positive and proactive way to daily handovers and Multi-Disciplinary Discussions
  • Work with individuals through trusting relationships to collaboratively develop and review person – centred support plans that focus on their goals and hopes, using DIALOG+ support tool and evidence-based interventions
  • Working alongside AWP, PCLS, and other professionals to identify appropriate pathways of support to achieve these using Your team, Your conversation, Your Plan assessments and other self-management tools

Requirements:

  • Experience working with people with mental health needs, or people with complex needs
  • Proven knowledge of support needs of people with mental health needs
  • Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions
  • A current, full driving licence and access to an appropriate vehicle, with business insurance is desirable
