Job Responsibility

To be responsible for the review and management of a caseload of patients who have substantial and complex needs
Provide specialist clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public
Provide high quality assessment and care, alongside offering the wider team support, advice and supervision
Work collaboratively with patients to develop care plans, recovery and wellbeing plans
Ensuring patients have access to regular reviews to monitor progress and adapt care plans as necessary
Consider carer needs and ensure carer and family are involved in care planning where appropriate
Deliver evidenced based clinical interventions
