Job Description:

We're looking for 2 proactive and skilled Reablement Assistants, who will be working within Slough's Reablement and Independence Service to support people in achieving increased independence via outcome based reablement plans designed by Occupational Therapists. Plans will include activity re-training, rehab programmes, practical support, and teaching people how to use equipment to aid independence. The role will be undertaken within a home setting or in the community depending upon each person's Reablement plan.
