The Respiratory Care Practitioner (RCP) Navigator plays a vital role in coordinating the safe and efficient transition of pediatric patients with respiratory needs from hospital to home or other care environments. Working across interdisciplinary teams, the RCP Navigator ensures appropriate equipment is provided, families are fully educated and confident in managing care, and respiratory plans are aligned with the patient's clinical and palliative goals. With deep expertise in pediatric respiratory therapies and durable medical equipment (DME), the RCP Navigator will serve as a clinical bridge between providers, case management, bedside care, and families. This position also supports the hospital's palliative care model by helping patients and families navigate complex care needs with compassion, clinical precision, and clear communication.
Job Responsibility:
Coordinate the safe and efficient transition of pediatric patients with respiratory needs from hospital to home or other care environments
Ensure appropriate equipment is provided
Ensure families are fully educated and confident in managing care
Ensure respiratory plans are aligned with the patient's clinical and palliative goals
Serve as a clinical bridge between providers, case management, bedside care, and families

Requirements:
Associates Degree Respirator Care (required)
Bachelors Degree Health Related Field (preferred)
RCP - Respiratory Care Practitioner License - RCB - Respiratory Care Board of California (required)
RRT - Registered Respiratory Therapist - NBRC - National Board for Respiratory Care (required)
Minimum three (3) years Experience in pediatric acute care (preferred)
Strong understanding of respiratory-related DME
Demonstrated ability to educate families and caregivers effectively
Exceptional interpersonal and communication skills
Familiarity with palliative care approaches is a plus
