Job Description:

Working as part of a team across Rockhampton QLD, you will enjoy autonomy in carrying out your work, traveling to aged care homes to conduct Residential Aged Care Funding Assessments using the AN-ACC tool. While direct care isn't part of the day-to-day work, you will use your expert clinical skills and knowledge to conduct interviews with residents and staff and review health records to inform the AN-ACC assessment.

Job Responsibility:

Requirements:

  • 5 years’ experience post graduate experience in aged care including working with people with dementia
  • Relevant tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy with unrestricted AHPRA registration
  • Current Influenza vaccination evidence
  • Ability to conduct clinical assessments using a range of assessment tools
  • Strong organisational skills and the ability to work independently and as part of a team
  • A genuine desire to improve the lives of older Australians
  • Your own vehicle as travel will be required within your region
