Are you a passionate and dedicated Social Worker looking for a challenging yet rewarding role in a dynamic healthcare setting? We're inviting applications for an exciting opportunity to join a Hospital Social Work Team within Fife. There are two crucial roles based either in the East and West teams, where you'll be at the forefront of providing timely, person-centered support to patients and their families.
Job Responsibility
Conducting holistic assessments of patients' needs in a timely manner, focusing on social, emotional, and practical factors influencing discharge
Developing and implementing person-centred discharge plans, ensuring safe and sustainable outcomes for patients
Coordinating community care services, including liaising with Fife Health and Social Care Partnership services
Providing counseling, support, and advocacy for patients and their families during critical periods of transition and crisis
Managing a complex caseload, prioritising effectively to meet hospital flow demands
Applying relevant legislation (e.g., Adults with Incapacity (Scotland) Act 2000, Social Care (Self-directed Support) (Scotland) Act 2013)
Requirements
A degree in Social Work or an equivalent qualification recognised by the Scottish Social Services Council (SSSC)
Current SSSC registration as a social worker
