Are you a passionate and dedicated Social Worker looking for a challenging yet rewarding role in a dynamic healthcare setting? We're inviting applications for an exciting opportunity to join a Hospital Social Work Team within Fife. There are two crucial roles based either in the East and West teams, where you'll be at the forefront of providing timely, person-centered support to patients and their families.

Job Responsibility

  • Conducting holistic assessments of patients' needs in a timely manner, focusing on social, emotional, and practical factors influencing discharge
  • Developing and implementing person-centred discharge plans, ensuring safe and sustainable outcomes for patients
  • Coordinating community care services, including liaising with Fife Health and Social Care Partnership services
  • Providing counseling, support, and advocacy for patients and their families during critical periods of transition and crisis
  • Managing a complex caseload, prioritising effectively to meet hospital flow demands
  • Applying relevant legislation (e.g., Adults with Incapacity (Scotland) Act 2000, Social Care (Self-directed Support) (Scotland) Act 2013)

Requirements

  • A degree in Social Work or an equivalent qualification recognised by the Scottish Social Services Council (SSSC)
  • Current SSSC registration as a social worker
