Job Responsibility:
Conducting statutory reviews for older adults in accordance with relevant legislation and local policies
Working in partnership with service users, carers, families, and multidisciplinary teams (MDTs) to assess ongoing needs and outcomes
Developing and updating care and support plans to reflect current needs, promoting independence, choice, and control
Identifying and responding to safeguarding concerns promptly and effectively
Maintaining accurate and timely records using HSCP systems
Requirements:
A degree in Social Work or an equivalent qualification recognised by the Scottish Social Services Council (SSSC)
Current SSSC registration as a social worker
