Job Responsibility

  • Conducting statutory reviews for older adults in accordance with relevant legislation and local policies
  • Working in partnership with service users, carers, families, and multidisciplinary teams (MDTs) to assess ongoing needs and outcomes
  • Developing and updating care and support plans to reflect current needs, promoting independence, choice, and control
  • Identifying and responding to safeguarding concerns promptly and effectively
  • Maintaining accurate and timely records using HSCP systems

Requirements

  • A degree in Social Work or an equivalent qualification recognised by the Scottish Social Services Council (SSSC)
  • Current SSSC registration as a social worker
