Job Description:

Are you a Qualified Social Worker looking for a new challenge within a Hospital setting? Randstad are recruiting Qualified Social Workers across Scotland. These are full time roles offering fantastic competitive pay rates.

Job Responsibility:

  • Discharge Planning & Logistics
  • Coordinating transfers to skilled nursing facilities, rehab centers, or hospice
  • Setting up home health care, physical therapy, or medical equipment (like oxygen or hospital beds)
  • Linking patients with community programs like "Meals on Wheels" or transportation services
  • Clinical Assessment & Intervention
  • Evaluating a patient's mental health, support system, and living situation
  • Providing immediate support for victims of domestic violence, sexual assault, or child/elder abuse (including mandatory reporting)
  • Identifying addiction issues and facilitating referrals to detox or recovery programs
  • Helping patients understand Medicare/Medicaid or apply for "charity care" for unpaid bills
  • Assisting with Power of Attorney (POA) and Living Wills so the patient's wishes are respected
  • Acting as a bridge between the medical team and the family when there is disagreement over the care plan
