The role of the Social Worker - Grade 2 is to contribute to excellence in patient care through the provision of professional social work services and to provide the highest standard of clinical input to the multidisciplinary treatment team.
Job Responsibility:
Provide professional social work services
Provide psychosocial assessment
Provide social casework (individual & family)
Provide supportive counselling
Provide consumer & carer advocacy
Conduct family work (including family meetings)
Conduct group work
Perform community liaison
Demonstrate specialist skills in assessment and discharge planning
Perform other interventions with aged people with mental health problems and their families/carers
Requirements:
Previous social work experience
Experience in the aged, mental health or older adult mental health sectors
Well-developed clinical expertise
Commitment to high standards of patient care
Demonstrated capacity to work effectively as a member of the multidisciplinary team
Minimum of two years' post-graduate social work experience in clinical settings
Nice to have:
Previous experience in mental health, aged care, or older Adult mental health regarded favourably
