Completed relevant post-qualifying training

Excellent communication and written skills
Experience of working in a multi-agency/inter disciplinary environment and knowledge of the functioning of the main agencies involved in this area of responsibility
Knowledge & experience of Adult Social Care Legislation and its application within the service
The ability to undertake Social Work duties and practice within legal, policy and procedural framework
The ability to manage own Social Work practice within the organisation
The ability to develop and maintain professional relationships within and outside the organisation

Job Responsibility

To be an integral member of a community social work team for adults aiming to provide a comprehensive assessment and case management service and to promote the highest standards of professional practice
To undertake the statutory and legislative functions including court facilitation around the arrangement of services to achieve the safe objectives within the policies of the department/organisation
To enable strength-based outcomes which promote community based social welfare and inclusion for individuals, carers, families, groups, and Communities
To contribute to inter-agency working by: Developing working relationships and sharing information
Supporting the co-ordination of services for individual cases
Developing knowledge of local arrangements, services and sources of advice for supporting and safeguarding adults
Undertaking specified tasks together with other agencies

Job Responsibility

Collaborate with children, families, and relevant professionals to gather information, develop rapport, and engage them in the assessment process, respecting their views and cultural backgrounds
Analyze assessment findings and formulate clear and concise reports, including recommendations for intervention and support plans, in accordance with statutory requirements and organizational policies
Liaise with external agencies, such as schools, healthcare providers, and law enforcement, to gather additional information and coordinate services to meet the identified needs of children and families effectively
Provide expert advice and consultation to colleagues, including social work assistants, team managers, and other professionals, regarding assessment processes and best practices
Maintain accurate and up-to-date case records, documentation, and electronic records, ensuring compliance with regulatory standards and data protection requirements
Participate in regular supervision, team meetings, and training sessions to enhance professional development and practice knowledge
