Job Description:

We have a Qualified Social Worker role available in the fantastic and supportive Transitions Team covering the whole county. In this role you will be part of a team working across Adults, Childrens and Special Education; working with young people, families, and schools to support the empowerment for young people. Your work will have a direct impact on people’s lives, ensuring that the voices of young people, their parents, and carers are heard. Your work will be person-centred based around the 4 pillars of preparing for adulthood.

Job Responsibility:

  • Be an ambassador and leader across social work, both internally and externally
  • Provide a high-quality and effective professional casework service, managing a caseload
  • Comply with legislation, statutory guidance, and local policies and procedures
  • Deliver outstanding support to residents and ensure regular monitoring and review of care and support plans
  • Carry out needs assessments
  • Create person-centred and strengths-based care plans
  • Ensure delivery of services and review outcomes
  • Work in partnership with other professionals and organisations
  • Endorse and act by the principles of personalisation

Requirements:

  • Qualified Social Worker (hold a social work degree)
  • Registered with Social Work England
  • Full driving licence with access to a vehicle
  • In-depth knowledge of legislation and statutory guidance
  • Familiar with working to a strength-based approach
  • Experience and skills in working with young people and their families
  • Resilience, persistence, supportive and adaptable approach
  • Competent in all areas of the Professional Capabilities Framework of Social Work
