We have a Qualified Social Worker role available in the fantastic and supportive Transitions Team covering the whole county. In this role you will be part of a team working across Adults, Childrens and Special Education; working with young people, families, and schools to support the empowerment for young people. Your work will have a direct impact on people’s lives, ensuring that the voices of young people, their parents, and carers are heard. Your work will be person-centred based around the 4 pillars of preparing for adulthood.
Job Responsibility:
Be an ambassador and leader across social work, both internally and externally
Provide a high-quality and effective professional casework service, managing a caseload
Comply with legislation, statutory guidance, and local policies and procedures
Deliver outstanding support to residents and ensure regular monitoring and review of care and support plans
Carry out needs assessments
Create person-centred and strengths-based care plans
Ensure delivery of services and review outcomes
Work in partnership with other professionals and organisations
Endorse and act by the principles of personalisation
Requirements:
Qualified Social Worker (hold a social work degree)
Registered with Social Work England
Full driving licence with access to a vehicle
In-depth knowledge of legislation and statutory guidance
Familiar with working to a strength-based approach
Experience and skills in working with young people and their families
Resilience, persistence, supportive and adaptable approach
Competent in all areas of the Professional Capabilities Framework of Social Work
