Job Description:

You will play a vital role in supporting adults facing mental health challenges. Your duties will include carrying out comprehensive assessments to understand individual needs, creating tailored care plans, and working with a range of services to ensure timely access to the right mental health support.

Job Responsibility:

Conduct comprehensive assessments to identify clients' mental health needs and develop appropriate care strategies
Perform mental capacity assessments in line with the Mental Capacity Act 2005, and make Deprivation of Liberty Safeguard (DoLS) applications as needed to ensure legal compliance and safeguard individuals' rights
Provide immediate support and intervention in response to mental health emergencies and crises
Collaborate with a multidisciplinary team, including psychiatrists, psychologists, nurses, and other healthcare professionals, to deliver coordinated, holistic care
Maintain accurate and timely records of assessments, interventions, and care plans, ensuring all documentation meets council policies and legal requirements

Requirements:

Relevant social work qualification
2 experience in adult social work, including assessment and care planning
